Введение. Профессиональные злокачественные новообразования (ЗНО) в структуре профессиональной патологии в нашей стране длительное время занимают самое последнее ранговое место, составляя не более 0,5%. Фактически, в год в Российской Федерации выявляется от 20 до 40 случаев профессионального рака. Доля ЗНО в структуре профессиональных заболевания, выявляемых в странах ЕЭС составляет около 5% (до 3700 случаев в год). Фактическое число выявляемых случаев профессионального рака в нашей стране ниже прогнозных оценок на несколько порядков. Проблема неполного выявления профессионального рака достаточно многогранна, во многом обусловлена и недостаточной настороженностью врачей первичного звена, высокой вероятностью длительного латентного периода заболевания и т. д.

Цель исследования — поиск возможных случаев профессионального рака на основе данных, о вновь выявленных случаях ЗНО, внесённых в региональный раковый регистр.

Материалы и методы

Материалы и методы. Объектами исследования стали 1359 первичных случаев ЗНО, внесённых в региональный раковый регистр на базе Забайкальского краевого онкологического диспансера. Был разработан алгоритм действий, на основании которого проведён отбор случаев с учётом локализации заболевания, анамнестических сведений, возрастно-стажевых параметров. Выделены случаи с подозрением на профессиональный характер заболевания, с последующим запросом документов, необходимых для проведения экспертизы связи заболевания с условиями труда. На базе Центра профпатологии Забайкальского края проведена непосредственно экспертиза, подведены итоговые результаты.

Результаты

Результаты. Из 1359 случаев них было отобрано 49 пациентов и после дополнительного запроса сведений о профессиональном маршруте, стаже работы в сопоставлении с нозологической формой ЗНО, выделено 16 случаев с подозрением на профессиональный характер заболевания, из которых в 12 случаях была установлена причинно-следственная связь развития заболевания с условиями труда, в т. ч. в 4 случаях у пациентов в постконтактном периоде.

Ограничения исследования

Ограничения исследования. Исследование ограничено количеством случаев онкологических заболеваний, предоставленных для анализа.

Заключение

Заключение. Полученные данные свидетельствуют, что на примере одного субъекта федерации за 10 месяцев работы по целенаправленному анализу данных ракового регистра, было выявлено число случаев профессионального рака, практически сопоставимое с количеством подобных заболеваний, выявляемых за год в целом по стране.

Introduction

Introduction. Occupational malignant neoplasms (OMN) in the structure of occupational pathology in our country for a long time occupy the last rank place, amounting to no more than 0.5%. In fact, in the Russian Federation, doctors detect from 20 to 40 cases of occupational cancer every year. The share of OMN in the structure of occupational diseases detected in the EEC countries is about 5% (up to 3,700 cases per year). The actual number of detected cases of occupational cancer in our country is several orders of magnitude lower than forecast estimates. The problem of incomplete detection of occupational cancer is quite multifaceted, largely due to the lack of vigilance of primary care doctors, the high probability of a long latent period of the disease, etc.

The study aims to find possible causes of occupational cancer based on data on newly identified cases of OMN entered into the regional cancer registry.

Materials and methods

Materials and methods. The objects of the study were 1,359 primary cases of OMN, entered into the regional cancer registry on the basis of the Zabaikalsky regional oncological dispensary. Scientists have developed an algorithm of actions, on the basis of which they carried out the selection of cases, taking into account the localization of the disease, anamnestic information, age parameters. The authors identified cases with suspicion of the occupational nature of the disease, followed by a request for documents necessary to conduct an examination of the connection of the disease with working conditions. On the basis of the Center for Occupational Pathology of the Trans-Baikal Territory, specialists conducted a direct examination and summed up the final results.

Results

Results. The authors selected 49 patients from 1359 cases and, after an additional request for information on the professional path, work experience in comparison with the nosological form of OMN, identified 16 cases with suspected occupational disease, of which a causal relationship between the development of the disease and working conditions was established in 12 cases, including 4 cases in patients in the post-contact period.

Limitations

Limitations. The study is limited by the number of cancer cases provided for analysis.

Conclusion

Conclusion. The data obtained indicate that, using the example of one subject of the federation, over 10 months of work on the targeted analysis of cancer registry data, the number of cases of occupational cancer was revealed, almost comparable to the number of similar diseases detected in a year in the whole country.
