Осимертиниб — ингибитор тирозинкиназы рецептора эпидермального фактора роста 3-го поколения — единственный на сегодняшний день препарат, зарегистрированный для лечения метастатического немелкоклеточного рака лёгкого у пациентов с позитивным статусом мутации T790M. На территории Российской Федерации планируется производство осимертиниба мезилата, в связи с чем возникла необходимость в проведении исследований по обоснованию гигиенического норматива содержания аэрозоля вещества в воздухе рабочей зоны.</p><p>Проведение исследований по обоснованию предельно допустимой концентрации вещества требует значительных материальных и временных ресурсов, при этом использование результатов доклинических и клинических исследований, математического моделирования процессов, методологии оценки риска позволяет получить комплексную оценку вещества, значительно сократив при этом сроки и стоимость проведения работ.</p><p>Цель исследования — научное обоснование предельно допустимой концентрации фармацевтической субстанции осимертиниба мезилата в воздухе рабочей зоны с использованием данных токсикологических исследований и математического моделирования воздействия вещества на человека в производственных условиях.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В качестве материалов были использованы отечественные и международные базы данных, отчёты, протоколы исследований, научные статьи и монографии, содержащие информацию о физико-химических и токсических свойствах, фармакотерапевтической активности осимертиниба.</p></sec><sec><title>Результаты</title><p>Результаты. Гигиеническую регламентацию осимертиниба проводили с учётом данных о его токсичности, опасности, фармакотерапевтической активности, побочных, отдалённых эффектов, фармакокинетических исследований и моделирования. Для обоснования величины ПДК использованы следующие реперные точки: недействующий уровень NOEL 1 мг/кг/день по общетоксическому действию, установленный в 92-дневном эксперименте на крысах при внутрижелудочном введении; наименьший наблюдаемый уровень неблагоприятного воздействия LOAEL 0,5 мг/кг/день по общетоксическому действию, установленный в 180-дневном эксперименте на собаках при внутрижелудочном введении; уровень, не вызывающий неблагоприятного воздействия NOAEL, 10 мг/кг/день по общетоксическому действию, установленный в 180-дневном эксперименте на мышах при внутрижелудочном введении. Безопасные концентрации осимертиниба в воздухе рабочей зоны оценены с использованием реперных точек, в том числе по фармакологическому эффекту и минимальной суточной терапевтической дозе; токсикокинетического моделирования концентраций вещества в организме человека в производственных условиях и уровня минимального риска.</p></sec><sec><title>Ограничения исследования</title><p>Ограничения исследования. Исследование ограничено изучением открытых литературных источников при описании токсикологических/токсикокинетических характеристик осимертиниба мезилата.</p></sec><sec><title>Заключение</title><p>Заключение. Наиболее жёсткие показатели безопасных уровней воздействия, установленные на основе фармакологического эффекта и токсикокинетического моделирования на собаках, позволили рекомендовать в качестве ПДК среднесменной в воздухе рабочей зоны для осимертиниба мезилата величину 0,005 мг/м3, аэрозоль, 1 класс опасности.</p></sec><sec><title>Этика</title><p>Этика. Osimertinib, a 3rd generation epidermal growth factor receptor tyrosine kinase inhibitor, is currently the only drug registered for the treatment of metastatic non-small cell lung cancer in patients with a positive T790M mutation status. The production of osimertinib mesylate is planned on the territory of the Russian Federation, which necessitates research to substantiate the hygienic standard for the aerosol content of the substance in the air of the working area. Conducting research to substantiate the maximum permissible concentration of a substance requires significant material and time resources, while using the results of preclinical and clinical studies, mathematical modeling of processes, and risk assessment methodology allows for a comprehensive assessment of the substance, while significantly reducing the time and cost of work.</p><p>The study aims to scientifically substantiate the maximum permissible concentration of the pharmaceutical substance osimertinib mesylate in the air of the working area using data from toxicological studies and mathematical modeling of the effects of the substance on humans in industrial conditions.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The materials used were domestic and international databases, reports, research protocols, scientific articles and monographs containing information on the physico-chemical and toxic properties, pharmacotherapeutic activity of osimertinib.</p></sec><sec><title>Results</title><p>Results. The specialists have conducted hygienic regulation of osimertinib, taking into account data on its toxicity, danger, pharmacotherapeutic activity, side effects, long-term consequences, pharmacokinetic studies and modeling. To substantiate the value of the maximum permissible concentration, the authors used the reference points: an inactive NOEL level of 1 mg/­kg/­day for general toxic effect, established in a 92-day experiment on rats with intragastric administration; the lowest observed level of adverse effects of LOAEL is 0.5 mg/kg/day for general toxic effect, established in a 180-day experiment on dogs with intragastric administration; the level that does not cause adverse effects of NOAEL is 10 mg/kg/day for general toxic effect, established in a 180-day experiment on mice with intragastric administration. Safe concentrations of osimertinib in the air of the work area were evaluated using reference points, including the pharmacological effect and the minimum daily therapeutic dose; toxicokinetic modeling of concentrations of a substance in the human body under production conditions and the level of minimal risk.</p></sec><sec><title>Limitation</title><p>Limitation. The study is limited to the review of open literature sources describing the toxicological/toxicokinetic characteristics of osimertinib mesylate.</p></sec><sec><title>Conclusion</title><p>Conclusion. The most stringent indicators of safe exposure levels, established on the basis of pharmacological effect and toxicokinetic modeling in dogs, allowed us to recommend 0.005 mg/m3, aerosol, hazard class 1 as the maximum permissible concentration in the air of the working area for osimertinib mesylate.</p></sec><sec><title>Ethics</title><p>Ethics.
