Ее возникновение обусловлено физическими перегрузками и функциональным перенапряжением мышц верхних конечностей, приводящими к травматизации периферических нервных волокон из-за натяжения или повреждения нервных окончаний в напряженной мышце в условиях выраженного снижения уровня кровотока. Изучение особенностей болевого синдрома при профессиональных полиневропатиях является важной задачей, решение которой может способствовать более эффективному лечению основных клинических проявлений этого заболевания.Обследованы 236 пациентов с установленным диагнозом профессиональной полиневропатии верхних конечностей в возрасте от 18 до 65 лет без значимой сопутствующей патологии. Исследование проводилось с помощью стандартных опросников (опросник для диагностики невропатической боли DN4, краткий болевой опросник, госпитальная шкала тревоги и депрессии).Анкетирование по опроснику DN4 выявило у 91,4% больных наличие невропатического компонента болевого синдрома. Количественная оценка боли с помощью краткого болевого опросника подтвердила высокую интенсивность боли в руках у пациентов, а также ее влияние на профессиональную деятельность и другие сферы жизни. Кроме того, у большинства обследованных лиц были обнаружены признаки субклинической и клинической тревоги и депрессии.Боли в руках у пациентов с профессиональной полиневропатией являются сложной проблемой из-за своей высокой интенсивности, многолетнего хронического течения и влияния на многие аспекты повседневной жизни пациента. Для заболевания характерен смешанный ноцицептивно-невропатический характер болевого синдрома. Это объясняет сохранение боли на протяжении многих лет после прекращения контакта с этиологическим вредным производственным фактором (физическими перегрузками) и низкую эффективность обычных курсов консервативной терапии. Стойкая симптоматика профессиональной полиневропатии верхних конечностей, подтверждающаяся клиническими и инструментальными методами диагностики, диктует необходимость поиска новых методов терапии, направленной на уменьшение жалоб пациентов и клинических проявлений заболевания.</p></abstract><trans-abstract xml:lang="en"><p>Pain in the hands is a key symptom of professional upper limb polyneuropathy. Its occurrence is caused by physical overloads and functional overstrain of the upper limb muscles, leading to traumatization of peripheral nerve fibers due to tension or damage to the nerve endings in the tense muscle in conditions of a pronounced decrease in blood flow.The aim of study was to study the features of pain syndrome in professional polyneuropathies is an important task, the solution of which can contribute to more effective treatment of the main clinical manifestations of this disease.We examined 236 patients with a diagnosis of occupational upper limb polyneuropathy aged 18 to 65 years without significant comorbidities. In addition, most of the examined individuals showed signs of subclinical and clinical anxiety and depression.Hand pain in patients with occupational polyneuropathy is a complex problem due to its high intensity, long-term chronic course and impact on many aspects of the patient’s daily life. The disease is characterized by a mixed nociceptive-neuropathic nature of the pain syndrome. This explains the persistence of pain for many years after the termination of contact with the etiological harmful production factor (physical overload) and the low effectiveness of conventional courses of conservative therapy.
