Проблема высокой смертности от сердечно-сосудистых заболеваний обусловлена в том числе бессимптомным атеросклерозом (АС), который длительно протекает скрыто и манифестирует серьезными сосудистыми катастрофами, что имеет особую значимость для лиц, работающих на производственных объектах с тяжелыми и опасными условиями труда.</p><p>Цель исследования — оценить факторы риска развития мультифокального АС и состояние сосудистой стенки у работников основных профессий угольных предприятий юга Кузбасса.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Всего в исследование включено 384 человека (мужчины), из них 266 работников угольных шахт юга Кузбасса и 118 человек, не занятых в угольной промышленности, в возрасте от 40 до 55 лет. Диагностика АС проводилась на ультразвуковой системе «Vivid E9» фирмы-производителя GE с использованием линейного датчика для измерения толщины комплекса интима-медия и визуализации атеросклеротических бляшек. Для выявления значимых факторов риска изучались антропометрические, анамнестические данные, показатели липидного и углеводного обмена, наличие артериальной гипертензии.</p></sec><sec><title>Результаты</title><p>Результаты. Значимых различий частоты мультифокального АС в виде увеличения толщины интима-медия более чем на 1 мм и/или наличия атеросклеротических бляшек в обеих группах не установлено: 60,2% у шахтеров и 62,3% у работников, не связанных с добычей угля (р=0,703); при этом процент выявления атеросклеротических бляшек в артериях оказался значимо ниже среди работников угольной промышленности — 46,9% против 60,5% среди лиц группы сравнения (р=0,016). Общими факторами риска для всех обследованных оказались артериальная гипертензия и повышенный уровень гликированного гемоглобина; в свою очередь, у шахтеров-угольщиков дополнительными факторами риска были повышенные значения липопротеидов низкой плотности и индекса талия-бедро, а также отягощенная наследственность по сердечно-сосудистым заболеваниям, а среди лиц, не занятых в угольной промышленности, — курение.</p></sec><sec><title>Заключение</title><p>Заключение. Наиболее значимыми факторами риска развития мультифокального АС у работников угольной промышленности оказались артериальная гипертензия, повышенные значения гликированного гемоглобина, липопротеинов низкой плотности и индекса талия-бедро, а также отягощенная наследственность по сердечно-сосудистым заболеваниям. Значимых различий по частоте развития АС в виде увеличения толщины комплекса интима-медия магистральных артерий и наличия в них атеросклеротических бляшек в обеих обследованных группах не установлено, однако процент выявления атеросклеротических бляшек оказался ниже среди шахтеров.</p><p>Авторы заявляют об отсутствии конфликта интересов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The problem of high mortality from cardiovascular diseases is caused, among other things, by asymptomatic atherosclerosis, which proceeds latently for a long time and manifests itself as a serious vascular catastrophe, which is of particular importance for people working at production facilities with difficult and dangerous working conditions.</p><p>The aim of the study was to assess the risk factors for atherosclerosis and the state of the vascular wall in the workers of the main professions of coal enterprises in the South of Kuzbass.</p></sec><sec><title>Material and methods</title><p>Material and methods. In total, the study included 384 people (men), of whom 266 were the workers in coal mines in the South of Kuzbass and 118 people who were not employed in the coal industry, aged 40 to 55 years. The diagnosis of atherosclerosis was carried out on the ultrasound system “Vivid E9” of the manufacturing company GE using a linear sensor for measuring the thickness of the intima-media complex and visualization of atherosclerotic plaques. To identify significant risk factors, anthropometric, anamnestic data, indices of lipid and carbohydrate metabolism, and the presence of arterial hypertension were studied.</p></sec><sec><title>Results</title><p>Results. Significant differences in the frequency of asymptomatic atherosclerosis in the form of an increase in intima-media thickness by more than 1 mm and / or the presence of atherosclerotic plaques in both groups were not found: 60.2% among the miners and 62.3% among non-coal mining workers (p=0.703); at the same time, the percentage of detection of atherosclerotic plaques in arteries was significantly lower among coal workers — 46.9% versus 60.5% among people in the comparison group (p=0.016). The common risk factors for all examined subjects were arterial hypertension and increased level of glycated hemoglobin; in turn, in coal miners additional risk factors were increased values of low density lipoproteins and waist-hip index, as well as burdened heredity for cardiovascular diseases, and among the individuals not employed in the coal industry it was smoking.</p></sec><sec><title>Conclusions</title><p>Conclusions. The most significant risk factors for atherosclerosis in coal industry workers were arterial hypertension, raised values of glycated hemoglobin, low density lipoproteins and waist-hip index, as well as burdened heredity for cardiovascular diseases.
